Viscosupplementation involves injecting hyaluronic acid into the knee joint to improve lubrication, cushioning and smooth joint movement in osteoarthritis.

Viscosupplementation is a medical treatment that involves injecting hyaluronic acid into a joint, most commonly the knee, to improve lubrication, cushioning and joint movement. Hyaluronic acid is a naturally occurring substance found in healthy joint fluid, where it helps absorb shock and allows bones to glide smoothly during movement.

In conditions such as osteoarthritis, the quality and quantity of natural hyaluronic acid decline over time. Viscosupplementation aims to restore these properties within the joint, helping to reduce pain and stiffness while improving mobility.

How does Viscosupplementation work for knee osteoarthritis?

Hyaluronic acid injections work through mechanical, biological and anti-inflammatory mechanisms within the joint.

After injection into the joint space, hyaluronic acid:

  • Improves joint lubrication, reducing friction during movement
  • Enhances shock absorption, particularly during weight-bearing activities
  • Helps calm joint inflammation that contributes to pain and stiffness
  • Supports the joint environment, allowing smoother and more efficient movement

Unlike painkillers, viscosupplementation does not mask symptoms alone. Instead, it improves the quality of the joint fluid, which may help restore more natural joint function over time.

Benefits of Viscosupplementation

Hyaluronic acid supplements natural joint fluid, helping to reduce friction, absorb shock and ease pain caused by knee osteoarthritis.

There are several advantages that the treatment offers, such as:

  • Reduces joint pain and stiffness
  • Improves joint mobility and walking comfort
  • Enhances shock absorption in weight-bearing joints
  • Non-surgical and minimally invasive
  • May delay the need for surgery in suitable patients
  • Well-tolerated with a favourable safety profile

Who is suitable for Viscosupplementation in Singapore?

Although viscosupplementation is a popular treatment option for osteoarthritis, it may not be suitable for everyone. A proper medical assessment is essential to determine whether this treatment is appropriate for your condition.

Factors of a good candidate

  • Mild to moderate knee osteoarthritis – individuals with early to mid-stage cartilage wear may benefit most, as viscosupplementation helps improve joint lubrication and cushioning when some cartilage is still present.
  • Persistent knee pain despite conservative care – patients who continue to experience discomfort after physiotherapy, activity modification or oral medications may find additional relief with hyaluronic acid injections.
  • Preference for non-surgical treatment – those looking to manage osteoarthritis symptoms without proceeding to surgery often consider viscosupplementation as part of a stepwise treatment approach.
  • Limited tolerance to long-term medications – patients who cannot take prolonged anti-inflammatory or pain medications due to side effects or medical conditions may benefit from a localised joint treatment.
  • Desire to stay active and mobile – individuals aiming to maintain daily function and participate in exercise, work or perform mobility-related tasks may use viscosupplementation to support joint movement and comfort.

Factors that make someone less suitable

  • Advanced or end-stage knee arthritis – severe cartilage loss with marked joint deformity or bone-on-bone changes often responds poorly and surgical options may be more appropriate.
  • Active joint or skin infection – injections are avoided if there is infection in or around the knee, as this increases the risk of complications.
  • Known allergy to hyaluronic acid products – patients with hypersensitivity to hyaluronic acid or injection components should not undergo viscosupplementation.
  • Significant knee instability or mechanical damage – conditions such as severe ligament injury or loose bodies within the joint may require surgical intervention rather than injection-based treatment.

What to expect during Viscosupplementation?

A thorough knee assessment ensures viscosupplementation is appropriate and helps tailor treatment to the severity and pattern of arthritis.

Knee viscosupplementation is a simple, minimally invasive procedure that is generally well tolerated. It is carried out in a clinical setting with careful attention to safety, comfort and accuracy at every stage.

The process usually involves the following steps:

  • Pre-treatment review before the injection, our doctor will confirm the diagnosis of osteoarthritis, review any recent scans or X-rays and check your medical history and medications. The treatment plan will be discussed and consent obtained, with time for you to ask questions.
  • Preparation of the knee the skin around the knee is thoroughly cleaned using sterile technique. The injection site is carefully identified to ensure the hyaluronic acid is delivered accurately into the joint space.
  • Pain minimisation measures a local anaesthetic may be applied or injected to reduce discomfort during the procedure. Most patients describe the sensation as pressure rather than sharp pain.
  • Injection of hyaluronic acid the hyaluronic acid is strategically injected into the knee joint.
  • Short observation period after the injection, you will rest briefly in the clinic while the medical team monitors for any immediate reactions. This also allows time to explain aftercare instructions.
  • Same-day discharge the procedure does not require hospital admission. You can return home shortly afterwards with clear guidance on activity modification, symptom management and follow-up arrangements.

Aftercare and recovery after Viscosupplementation

Aftercare is important to optimise treatment response and reduce side effects. It typically requires you to:

  • Short period of rest – rest the treated knee for the first 24 hours after the injection to allow the hyaluronic acid to settle within the joint space and reduce irritation.
  • Temporary activity modification – avoid strenuous exercise, running, jumping or heavy lifting for a few days. Low-impact activities such as walking are usually acceptable unless advised otherwise.
  • Mild post-injection discomfort – slight swelling, stiffness or soreness around the knee may occur and typically resolves within 24 to 48 hours as the joint adapts to the injection.
  • Cold therapy, if needed – applying an ice pack for short intervals can help ease swelling or discomfort. Ensure ice is wrapped and not applied directly to the skin.
  • Gradual return to normal activities – most patients can resume usual daily activities within a short time, with progression guided by symptoms and medical advice.
  • Follow-up and monitoring – our doctor may schedule a review to assess response to treatment and advise on further injections or complementary knee rehabilitation if needed.

What results can you expect?

By reducing knee pain and stiffness, viscosupplementation can support walking, exercise and daily activities with greater comfort and confidence.

It is essential to recognise that viscosupplementation may not provide instant relief for everyone. The results vary depending on the severity of joint degeneration, activity levels and overall joint health.

Many patients experience gradual improvements, such as:

  • Reduced pain and stiffness within 2 to 4 weeks
  • Improved walking comfort and daily function
  • Benefits that may last several months

How many sessions are typically needed?

The number of injections depends on the severity of the degeneration and individual response. But generally:

  • Some conditions require a single injection
  • Others involve a course of 2 to 3 injections, spaced weekly

Our doctor will recommend the most suitable regimen based on your condition and response to treatment.

Can Viscosupplementation treat conditions other than osteoarthritis?

Viscosupplementation is most commonly used for osteoarthritis, particularly of the knee.
In selected cases, it may also be considered for:

  • Early cartilage wear
  • Post-injury joint irritation
  • Joint pain related to reduced lubrication rather than structural damage

However, our doctor will conduct a thorough evaluation to determine whether the condition can be treated with viscosupplementation. It should be noted that it is not a treatment for ligament tears, fractures or inflammatory arthritis and should always follow a proper diagnosis.

Summary

Early treatment for knee osteoarthritis helps slow joint deterioration, manage symptoms effectively and preserve long-term knee function.

In Singapore, viscosupplementation is commonly used as part of a stepwise osteoarthritis management plan before considering surgical options. It is a non-surgical treatment designed to restore lubrication and cushioning within the joint, particularly for knee osteoarthritis. By improving the quality of joint fluid, it can help ease pain, improve movement and support day-to-day activities with greater comfort.

What is A Locked Knee?

When the meniscus or cartilage in the knee is damaged, it may get stuck in your knee joint, making it difficult to move your knee. This is called a locked knee. Symptoms of a locked knee include:
  • Pain in your knee
  • Swelling in knee joint
  • Stiffness
  • A popping sound during movement
The most common cause of a locked knee is a tear in the meniscus or cartilage. Building muscle strength in your legs and knees can prevent a locked knee.
